Employee Assistance Program (EAP):

Employee Assistance is available 24/7 as a free and confidential support for all of our employees. Please contact HR for more information in regard to how to access this benefit as an employee of CCSD.

Employee Assistance Program Overview

Employee Injuries at Work: Silver Lining Nurse Advantage Triage

Step 1: Immediately notify a supervisor of the incident. If this is a life or limb threatening injury, call 911.

Step 2: Supervisor and injured employee should call (844) 891-6022

Step 3: A triage nurse gathers pertinent information and guides the injured employee to the appropriate care.

Step 4: If professional medical treatment is necessary, the triage nurse offers tele-medicine or in-person medical facility options.

Step 5: Submit paperwork to HR within 24 hours

When a call comes into Silver Lining Nurse Advantage, it is immediately routed to a West Bend nurse who works with the supervisor and injured employee to assess the injury. Using a comprehensive injury decision tree, the nurse will guide the injured employee to the appropriate care protocol, which could be selfcare, professional medical care provided at an offsite medical facility, or a tele-medicine consultation. If self-care is indicated, self-care instructions are proved to the injured employee and employer. If the injured employee opts for care at an offsite medical facility, the nurse has access to the applicable PPO network to help the employee get access to the appropriate care. All calls to a nurse are recorded.
